Can cabinets be painted well, or does it always look like painted cabinets?

The work is in the preparation: degreasing decades of cooking residue, sanding to a key, priming with a bonding primer, and spraying rather than brushing. Brushed cabinet doors always read as brushed cabinet doors. Is a refresh worth it before selling?

Usually, if the kitchen is dated rather than broken. What are the signs a refresh will be wasted money?

Those are structural or functional problems, and finishes do not touch them.

How disruptive is it?

Far less than a remodel. Expect the sink out of service for a day, the cabinets doorless for several days while the fronts are finished, and dust from the backsplash work. Most households cook through it with minor inconvenience rather than moving out of the kitchen entirely.
